OverviewOccupational Therapist - Primary Mental Health

role at

Rotherham Doncaster and South Humber Foundation Trust . Exciting opportunity to join our Primary care integrated mental health hub as a fixed term, 6 month Band 5 Occupational Therapist. Based in Rotherham, you will work with patients out in the community, assessing and visiting them in their own homes. You will be an autonomous practitioner with your own caseload of clients. You will assess the occupational and vocational needs of service users enabling them to develop independent living skills and work towards individual recovery goals.

Responsibilities

Work with patients in community settings, assessing and visiting in their own homes.

Act as an autonomous practitioner with your own caseload.

Assess occupational and vocational needs to support development of independent living skills and recovery goals.

Refer individuals to other professionals or agencies as appropriate (e.g., Social Services).

advocate for service users and provide support and advice to their family and carers as appropriate.

Liaise closely with other professionals involved in service user care across the RDaSH footprint on clinical issues.

Follow all Trust policies and protocols.

Work without direct supervision, manage time and caseload, and prioritise workloads.

Deliver clinical work in a variety of settings (ward-based, service user’s home, community) and comply with setting-specific rules and regulations.

Manage clinical information in line with service and Trust information requirements.

Contribute to departmental meetings and routines; document all interventions in patient records according to professional standards and local guidelines.

Qualifications / Requirements

Broad knowledge of current theory and best practice in occupational therapy.

Knowledge of principles of a holistic, person-centered approach to care; confident and effective communicator with service users, carers and staff across professional groups and with external partner agencies.
