We are seeking an individual who is an experienced leader, enthusiastic, driven and passionate about service transformation and delivery of change. This is a fantastic time to join our SALT service as it is currently undergoing excellent service transformation and change management. This post is full time with a large proportion of hybrid working.Responsibilities

Deliver the clinical service in the defined area and provide team leadership and management to the team.Provide leadership, management, and communication to the team to ensure efficient, effective, coordinated and high-quality care to patients/clients.Ensure delivery of the quality strategy within the designated area by establishing systems and processes.Provide clinical leadership and management to the clinical team and be responsible for the delivery of the service within the area.Work in partnership with other services/stakeholders to enable patients/clients to be treated in the appropriate setting.Ensure the team provides a high-quality service through caseload and clinical supervision; hold full line management responsibility for the team with appropriate delegation and delivery of patient/client care.Promote proactive teams delivering anticipatory and maintenance care, providing a responsive service for planned and unplanned needs.Maintain clinical credibility by providing direct clinical care and supporting team members in the clinical environment.Participate in caseload management development across the local health economy and provide leadership and mentoring to staff developing into caseload management roles.Act as an advocate and champion for patients/clients in various forums and professional groups, challenging attitudes and behaviour.Implement plans for rotas, schedules and working patterns to ensure business continuity; practice autonomously with evidence-based clinical decision making.Provide clinical expertise and knowledge to the team when managing complex and highly complex situations; assess patient/client conditions and present options when delivering complex care.Collaborate with stakeholders to deliver services and develop individualised care plans with patient, client and carer involvement; evaluate care delivery for identified needs.Align clinical activity with core objectives of health promotion, supported self-care, disease management, long-term condition management and end-of-life care.Set objectives and monitor performance; participate in local service meetings and report staff and patient activity as required.Support practice development, service modernisation, policy development and protocol authoring; contribute to role and service redesign.Provide induction and safety/operational policies for new team members and students; act as an authorised signatory ensuring probity in timecards and mileage claims.Monitor budgets, undertake personal development plans (PDPs), maintain Knowledge and Skills Framework (KSF) reviews and ensure mandatory training for all staff.Delegate administrative duties to clerical staff; provide data to monitor team contracts and objectives; participate in audits and research; supervise individuals and groups.Implement mentorship and clinical supervision; ensure consistent record keeping and maintain clinical standards; initiate training and development of team members.Identify risks, manage and report incidents per Trust policies; monitor patient care quality using audits, caseloads and supervision; participate in patient satisfaction reporting.Ensure staff mentoring for students and identify skills deficits with plans to address them to support service delivery and development.Qualifications/Requirements
