to join our multidisciplinary team and help us provide safe, high-quality care to our patients.
Location:
Opportunities available at sights across South East London Kingfisher and Surrey Docks (SE8 5DA); Penrose Surgery and Maddock Way (SE17 3DW), Lewisham Medical Centre (SE13 5PJ), Deptford Surgery (SE14 6TJ), Knight''s Hill Surgery (SE27 0DF) and Nightingale Surgery (SE12 8NP).Hours:
Ideally full time (weekdays)Key Responsibilities
Managing long-term conditions (e.g. diabetes, asthma, COPD) through routine reviews and proactive care, conducting chronic disease clinics delivering patient care and making referrals to specialist / secondary care where requiredProviding cervical cytology with subsequent result processingRoutine nursing care according to clinical based evidence, NICE and NSF guidelinesAdminister childhood immunisations, travel vaccines and flu jabsRequesting necessary pathology servicesProvide contraceptive advice and supportContributing to QOF and local incentive schemesPromoting health education, self-management and preventative careWorking closely with GPs, HCAs, Nursing Associates, and Pharmacists to deliver joined-up careEnsuring high standards of infection control and clinical documentationSupport Quality and Outcomes Framework (QOF) delivery and other enhanced services
Skills, Knowledge and ExpertiseMinimum requirements: You must have a current NMC registration, and we would ideally be looking for someone with experience working in a GP Primary Care setting. Previous experience working with EMIS also preferred but not required.
