Senior Practice Nurse Prescriber | East London NHS Foundation Trust

The Greenhouse is an inclusion practice managed by East London Foundation Trust. We register adult patients who are experiencing homelessness (this encompasses street homeless, hostel dwellers, those in temporary accommodation, and those who have no secure housing). Patients registered are offered the full range of primary care services. We also provide an outreach service in Hackney and the City, providing clinics in hotels, hostels, and shelters, looking after vulnerably housed service users and rough sleepers. The role is based in both the practice (1 day) and the outreach hotel settings (2 days).Responsibilities include:The post holder is responsible for ensuring the delivery of safe and effective nursing care to the practice population.The post holder is accountable for nursing service delivery and will lead and manage the Health Care Assistant.Clinically, the focus is on delivering evidence-based care for patients with long-term conditions, wound care and management, and preventative nursing interventions.As an autonomous practitioner, the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making.They will demonstrate safe, clinical decision-making and expert care within general practice.The post holder will work collaboratively with the multi-disciplinary team to meet patient needs, support policy and procedures, and provide leadership for the nursing team.Build effective working relationships with other healthcare professionals.Work within the team to deliver excellent primary healthcare services.Our mission is to make a positive difference to people''s lives by improving quality of life. We seek individuals who embody these qualities when supporting service users, carers, colleagues, and partner organizations.Additional duties include:Promote health and well-being through health promotion, education, screening, and therapeutic communication.Manage and treat long-term conditions following policies and guidelines.Review medication for effectiveness and support adherence.Prioritize health problems and intervene appropriately, including emergency care.Support women with family planning and cervical screening.Provide sexual health screening, signposting, and referrals.Manage blood-borne virus screening and vaccination programs.Implement vaccination and immunization programs for all ages.Offer travel health advice.Deliver wound care in partnership with local services.Diagnose and treat minor illnesses.Prescribe medication as an independent prescriber within legislative and policy frameworks.Communicate health status clearly using appropriate terminology and technology.Provide safe, evidence-based, cost-effective, personalized care.Work within practice guidelines, policies, and protocols.Maintain accurate records, considering data protection principles.Serve as a resource for advice on emotional and behavioral issues.Support interventions, consultation, and psychoeducation for prevention and resilience.Participate in achieving QOF and local service targets.This advert closes on Wednesday, 3 September 2025.
