The Greenhouse is an inclusion practice managed by East London Foundation Trust. We register adult patients who are experiencing homelessness (this encompasses street homeless, hostel dwellers, those in temporary accommodation, and those who have no secure housing). Patients registered are offered the full range of primary care services. We also provide an outreach service in Hackney and the City, providing clinics in hotels, hostels, and shelters, looking after vulnerably housed service users and rough sleepers. The role is based in both the practice (1 day) and the outreach hotel settings (2 days).Responsibilities include:The post holder is responsible for ensuring the delivery of safe and effective nursing care to the practice population.The post holder is accountable for nursing service delivery and will lead and manage the Health Care Assistant.Clinically, the focus is on delivering evidence-based care for patients with long-term conditions, wound care and management, and preventative nursing interventions.As an autonomous practitioner, the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making.They will demonstrate safe, clinical decision-making and expert care within general practice.The post holder will work collaboratively with the multi-disciplinary team to meet patient needs, support policy and procedures, and provide leadership for the nursing team.Build effective working relationships with other healthcare professionals.Work within the team to deliver excellent primary healthcare services.Our mission is to make a positive difference to people''s lives by improving quality of life.
