Job DescriptionJob Title: Care CoordinatorLocation: WestminsterSalary: £28,000- £30000Hours: 40 hours per weekReports to: Registered Manager / Operations ManagerJob PurposeThe Care Coordinator is responsible for the effective coordination and delivery of high-quality, person-centred care within domiciliary and extra care services. The role ensures service users receive safe, responsive, and well-planned care that promotes independence, dignity, and wellbeing.Key ResponsibilitiesService CoordinationCoordinate and manage day-to-day care service delivery.Develop and maintain staff rotas to ensure adequate care coverage.Allocate care workers based on skills, experience, and service user needs.Ensure continuity of care wherever possible.Care Planning and AssessmentsConduct service user assessments.Develop, review, and update comprehensive care plans.Complete and maintain detailed risk assessments.Ensure care plans are person-centred and outcome-focused.Compliance and Quality AssuranceEnsure services meet regulatory standards and organisational policies.Maintain accurate and up-to-date documentation and records.Monitor care delivery and address any quality concerns promptly.Support audits, spot checks, and service reviews.Staff Support and SupervisionSupport care staff with guidance and daily coordination.Identify training needs and escalate to management where necessary.Assist with induction of new staff members.Communication and LiaisonAct as the main point of contact for ..... full job details .....
