Care Coordinator
Cityworx are recruiting for an experienced Care Coordinator to join an established complex care business based in the Southwark, London area. As a Care Coordinator your role is to coordinate and oversee high-quality, person-centred care for individuals with complex needs. Acting as the central point of contact between clients, families, and multidisciplinary teams, the Care Coordinator ensures that care is delivered seamlessly, compassionately, and in line with best practice.
