Provide a comprehensive clinical pharmacy service to paediatric in-patientsProvide highly specialised paediatric drug therapy advice to patients/carers, medical and nursing staff and other healthcare professionalsProvide teaching to pharmacy, nursing and medical staff in specialist areas of clinical practice and supervises less experienced band 6 pharmacists and students.Main duties of the job

Provide a highly specialist clinical pharmacy service to paediatric inpatients including in accordance with professional, national, departmental and Trust policies.Undertake Medicines Reconciliation, medication review, formulation and review of pharmaceutical care plans and all other aspects of clinical and Near Patient PharmacyAdvise on drug interactions, adverse drug reactions, therapeutic drug monitoring and compatibilities of drugs ensuring a holistic approach to pharmaceutical care when advising on dose changes. Record interventions and clinical advice in patient care plans or medical case notes.Produce specialist drug therapy guidelines which impact on other disciplines using computer knowledge and keyboard skillsParticipate in daily multidisciplinary ward rounds and offer pharmaceutical advice.Actively contribute at specialist clinical patient discussion meetings by providing highly specialist pharmaceutical advice.Guide and advise medical and nursing staff in the choice of therapy, supportive therapy, safe prescribing, and suitable methods of administration and formulation of drugs.Provide and interpret highly specialist pharmaceutical information, in an area where potentially little or no information is available, to consultants, other medical and nursing staff and patients.Liaise with senior medical and nursing staff, including Advanced Nurse Practitioners, to plan therapies in advance of treatment.Work as a non-medical prescriber in a designated area of clinical practice or work towards this qualification.Consider the information needs of children with specialist conditions and provide written and verbal information for patients and parents.Facilitate the implementation of local and national clinical trials.Assist in audit of specialist therapies.Investigate, document and report medicines-related incidents or complaints exploring ways of minimising the risk of future re-occurrence.Assist in developing shared-care packages, liaising with appropriate healthcare professionals within shared-care hospitals to ensure the efficient and effective communication of uniform information across the secondary / tertiary interface.Support and develop the prescribing and administration of specialist therapies during day-case admissions and at home ensuring the pharmaceutical needs of patients are met and that there are the appropriate governance arrangements in place to ensure this treatment is managed safely and effectively.Ensure the cost effective use of drugs in specialist conditions.Communicate with consultants and other medical staff over prescription anomalies to ensure the safe administration of highly complex therapies.Monitor patients for toxicity, documenting, reporting and advising medical staff on adverse effects and their management.Participate in the evaluation of new drugs and supportive treatments to allow their safe introduction.Complete new drug request submissions for specialist treatments with supporting evidence required by DTC or CDEG.Take responsibility for the operation and development of specific specialist pharmacy services.Deputise for band 8a advanced pharmacistsProvide final product check support to the senior pharmacist in the Aseptic Unit when required with occasional exposure to unpleasant working conditionsProvide clinical training in all aspects of prescribing, including highly specialist therapies. This includes providing training to pharmacy, nursing and medical personnel.Regularly supervise and advise junior pharmacists and students. Tutor pharmacist on the clinical diploma if required.Educate parents and patients on the safe use of medication using developed communication skills when dealing with children/parents with language difficulties.Provide advice and information in distressing circumstances on PICU/HDU/Oncology when required.Participate in Continuing Professional Development as per department policy and regulations of the General Pharmaceutical Council.Participate in rotas to provide pharmacy services at weekends, for late clinics and bank holidays.Provide an out of hours paediatric on-call pharmacy service as outlined in the Medicines Management Code and Pharmacy On-call pathwayParticipate in flexible hours of working between the hours of 8am and 6pm.Person specification
