Patient facing long term condition clinics: Carry out consultations with patients with single or multiple medical conditions where medicine optimisation is preferred. Review the ongoing need for each medicine, review monitoring needs and support patients with their medicine taking to ensure optimal use. Make appropriate recommendations to GPs for medicines improvement.Patient facing clinical medication reviews: Undertake clinical medication reviews with patients and produce recommendations for GPs on prescribing/monitoring. Patient facing Care Home Medication Review: Undertake clinical medication reviews with patients and produce recommendations for GPs on prescribing and monitoring. Work with care home staff to improve safety and appropriate medication ordering (medication waste).Patient facing domiciliary clinical medication review: Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacist, nurses and GPs on prescribing and monitoring. Attend and refer patients to multidisciplinary case conference.Management of common/minor/self-limiting ailments: Manage caseload of patients with common/minor/self-limiting ailments within scope of practice and limits of competence. Signpost to community pharmacy and refer to appropriate healthcare professional (e.g., GP/Practice Nurse/Advanced Nurse Practitioner).Telephone medication support: Provide telephone support for patients with queries or concerns about their medication.Management of medications at discharge from hospital to reconcile medications upon discharge from hospitals, immediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medication supplies to high-risk groups of patients.Medicines information to practice staff and patients: Answer relevant medication queries from GPs, other network staff, other healthcare teams (e.g., community pharmacy) and patients with queries about medications. Suggest and recommend solutions. Provide follow up with patients to monitor the effect of any changes.Unplanned hospital admissions: Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.Signposting: Ensure patients are referred to the appropriate healthcare professional for the appropriate level of care in a timely manner (e.g., pathology results, common/minor ailments, acute conditions, long term condition review).Repeat prescribing: Adhere to local networks repeat prescribing policy. Manage the repeat prescribing and re-authorisation process by reviewing patient requests for repeat medications and review medication review dates and arranging reviews. Ensure patients have the appropriate monitoring tests in place when required.Risk stratification: Identify cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches.Service development: Contribute pharmaceutical advice for the development and implementation of new services with medicinal components (e.g., advice on treatment pathways and patient information leaflets).Information management: Analyse, interpret and present medicines data to highlight issues and risks to support decision making.Medicines quality improvement: Undertake clinical audits of prescribing in areas directed by the GPs, provide feedback on results and implement changes with the relevant practice team.Medicines safety: Implement changes to medicines that result from MHRA drug safety alerts.Implementation of local and national guidelines and formulary recommendations: Monitor practice prescribing against the formulary and make recommendations to GPs for medicines that should be prescribed by hospital doctors or subject to shared care. Work with the local medicines management team to ensure practice compliance to local formulary.Education and training: Provide education and training to the primary healthcare team on therapeutic and medicines optimisation.Care Quality Commission: Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.Public Health: Support public health campaigns and provide specialist knowledge on public health programmes available to the general public.Collaborative working arrangements: Participate in the PCN MDT meetings.Qualifications and job details
