Highly Specialist LD/Autism Practitioner

Overview
This role is part of exciting new developments across North Central London aimed at enhancing the existing mental health crisis services for young people with autism and/or learning disabilities (LD) and their families. The aim is to avoid unnecessary admissions to mental health inpatient units and to reduce the length of hospital stays. A Specialist LD/Autism Practitioner is being recruited for each of the 5 NCL boroughs — Camden, Islington, Barnet, Enfield and Haringey. This post will be based in Islington within the Islington CAMHS Adolescent Assessment + Outreach Team, working with CYP with acute, complex and chronic mental health difficulties. The post holder will interface with local sector inpatient units, acute paediatric wards and Emergency Departments, Home Treatment Teams, LD/ASD Keyworker services and CAMHS neurodevelopmental pathways to enhance local community support, improve discharge planning, and reduce inpatient stays for CYP with autism and/or LD.The post holder will be case holding, provide direct clinical interventions to a small caseload, work individually with young people and their families with diagnosed autism and/or LD, and deliver specialist, adjusted support and interventions, which may include Positive Behaviour Support (PBS)-based interventions. The role includes advocacy and support for CYP and families in navigating local systems and services (e.g., CETRs, social prescribing) and will involve indirect work such as consultation to residential units, upskilling other CAMHS clinicians in PBS, and coordinating management of behaviours that challenge.Initial discussions and planning will involve working as part of the AAOT MDT to consider clinical needs of CYP who may be on the local DSR, or who may present in crisis requiring psychiatric inpatient admission.Main duties
1. Clinicala) Provide comprehensive, specialist mental health assessments, including risk assessments, of young people with autism and/or LD who are considered at risk of escalating into mental health crisis and/or inpatient admission.b) Hold a caseload of CYP with complex, high-risk mental health needs where there is also a diagnosis of autism and/or LD; act as case manager and provide specialist, adjusted interventions including PBS-based interventions and family work.c) Offer joint working to other AAOT clinicians to support their work with CYP and families with autism and/or LD.d) Work as part of the AAOT MDT to consider clinical needs of CYP who may be on the local DSR, or who may present in crisis and require inpatient admission.e) Provide specialist consultation to CAMHS clinicians and professionals to aid developing shared formulation and system-based thinking for complex case management and to support development of adjusted management, care, crisis and safety plans.f) Provide specialist case consultation to Paediatric Mental Health Teams, inpatient units, Home Treatment Teams, Keyworker Services, etc., to support intensive crisis responses and to aid complex discharge processes and reduce admissions and length of inpatient stays.g) Attend and feed into the local Dynamic Support Register (DSR) and Islington Multiagency Planning and Support (IMAPS) panel.h) Ensure the needs of the client group are paramount and adhere to Islington Safeguarding Children’s Board (ISCB) Guidelines relating to Safeguarding Children.i) Provide reports and communicate in a skilled and sensitive manner concerning the assessment, formulation, treatment plans and needs of clients.j) Liaise with health, social care and education staff from a range of agencies in the care provided to clients from each service.k) Carry out clinical work in health and education settings and in clients’ homes to best meet their needs and in line with service practice.2. Teaching, Training, Consultation, Supervisiona) Provide training on neurodevelopmental conditions and LD and their impact, making adjustments and discussing treatment and support options with key agencies, services, teams and professionals; including management of behaviours that challenge.b) Provide consultation to staff in residential units.c) Provide consultation and training to upskill other CAMHS crisis team clinicians in PBS to enable a common language of PBS across teams.d) Develop group and teaching possibilities for parents/carers on psychoeducation and strategies to manage challenging behaviours.e) Act as clinical supervisor to AAOT or other CAMHS clinicians, assistants, students and/or trainees, as appropriate, in agreement with line-manager.f) Attend regular clinical and line management supervision in line with CAMHS and professional guidelines.g) Participate in appraisals in line with CAMHS and Trust Appraisal Policy.h) Maintain high personal standards of practice through active participation in internal and external CPD, in consultation with the line manager.3. Management, Policy and Service Developmenta) Work with NCL ICB project leads to monitor and report on the project against NCL agreed metrics.b) Advise clinicians and staff across the Trust and partner agencies in neurodevelopmental disorders, non-medical prescribing, and management of behaviours that challenge; help develop standards underpinned by theory and research.c) Participate in developing system-wide policies and guidelines as a Senior CAMHS Clinician and translate these into partnership working with services within and outside the Trust (e.g., NCL ICB, locality CAMHS, Adult Community Mental Health Teams, and other key agencies including Children’s Social Care).d) Contribute to the development, evaluation, implementation and monitoring of operational policies of Community CAMHS and AAOT for CYP with autism and/or LD.4. Clinical Governance, Research and Service Evaluationa) Participate in research undertaken within the service and disseminate findings; implement research in clinical practice.b) Maintain up-to-date knowledge of legislation, policies and issues relevant to the service and client group (CYP with autism/LD).c) Use theory, evidence-based literature and research to support evidence-based practice in individual and family work and with inter-agency colleagues.d) Lead as appropriate in audit and research projects and coordinate these within the team and/or service.Person Specification
