We''re looking for a passionate Health and Wellbeing Practitioner to join our team at HMP The Mount, supporting individuals on their journey towards recovery, better mental health, and a positive future beyond custody. This is a full-time position (35 hours per week) In this rewarding and varied role, you ll deliver person-centred support using both harm reduction and abstinence-based approaches. Working as part of an integrated healthcare team, you ll support individuals experiencing substance misuse issues, low-level mental health needs, and wider health and wellbeing challenges, helping them build resilience and prepare for release back into the community. What you ll be doing You ll play a key role in delivering high quality interventions and support, including: Managing a caseload and delivering personalised support and care plans Completing assessments, risk reviews and structured interventions Delivering 1:1 sessions and group programmes on substance misuse and low-level mental health topics Providing harm reduction advice and health and wellbeing guidance Supporting service users with release planning and continuity of care into the community Working collaboratively with healthcare, probation, housing, employment and external recovery services Maintaining accurate case records, ..... full job details .....
