You will work in Supported Living Services which provide 24 hour care and support to older people with learning disabilities and mental health. The role involves providing day to day support with daily living tasks, activities and making full use of community opportunities. Providing personal care and using hoists to assist with moving and handling as some service users have mobility issues. As part of the care and support team you will assist in maintaining a safe and enabling environment that provides positive experiences to those living within the service. Successful applicants will be: Providing support to individuals in line with agreed support plans Coordinating packages of care and support for individuals and supporting individuals with reviews of support and any needs for long term care or support.
