This is an ongoing contract position offering competitive pay at 38.18 per hour. You will work with children and young people who have disabilities and additional needs, supporting them and their families to improve outcomes and ensure their wellbeing and safety.Key ResponsibilitiesConduct thorough assessments of children with disabilities, considering their individual needs and circumstances.Develop and implement robust care and support plans tailored to each child and family.Work collaboratively with families, carers, health professionals, education providers, and other stakeholders to coordinate support services.Monitor the progress of children and families, regularly reviewing plans and adjusting support as necessary.Advocate on behalf of children with disabilities to ensure access to appropriate services and resources.Attend and contribute to multi-agency meetings, safeguarding conferences, and case reviews.Ensure all work complies with relevant legislation, policies, and safeguarding procedures.Maintain accurate, detailed records and prepare reports as required.Support children to have their ..... full job details .....
