The Role As a Care Coordinator, you will play a vital role in the smooth day-to-day running of the service. You will be responsible for scheduling care visits, supporting care staff, liaising with clients and families, and ensuring continuity of care across the community.Although this is an office-based role, a driving licence and access to a vehicle are essential to support occasional field visits, assessments, and cover when required.
