This role has a starting salary of -33,552 per annum, based on a 36 hour working week.

We are excited to be hiring 2 new Reablement Team Leaders to join our fantastic Reablement Service. The posts will be based in Camberley, working across the Surrey Heath area.

The roles will include both office-based and remote working.

About the Team

The aim of the Reablement Service is to enable individuals to stay living in their own homes for as long as possible. The service works with individuals for a period of up to six weeks to help them achieve their potential and to recommend onward referral for any future care needs.

The Reablement service works with local health and social care providers to enable residents to maximise their levels of independence either following a spell in hospital or prevent an unnecessary admission to hospital.

About the Role

As a Team Leader you will be responsible for coordinating the day-to-day work of the team in order to deliver a high quality service in accordance with regulatory requirements. This will include triaging all cases needing reablement support and managing a small team of Reablement Assistants and Integrated Reablement Workers to support people to achieve their support plan goals. You will work part of your hours on a rolling weekend rota and will work closely with Team Leaders in other areas to supporting the wider Reablement Service and co-ordinate staff rotas.

Shortlisting Criteria

To be considered for shortlisting for this position, your application will clearly evidence the following:

  • Experience in developing and reviewing reablement programmes that promote independence.
  • Proven ability to manage and support a staff team effectively.
  • Skilled in planning, prioritising and managing a caseload, with confidence to seek guidance when needed.
  • Flexible approach to working across different areas and building relationships with local partners.
  • Proficient in using a range of IT systems to support service delivery.
