You will carry out strength-based assessments under the Care Act 2014 and Mental Capacity Act 2005, focusing on safe discharge planning with a Home First ethos. Collaborating with health professionals, you will identify and arrange appropriate support, whether returning home, accessing rehabilitation, or further assessment in a bed-based setting.
