The

Hospice Social Worker

will work as a part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through NVNA and Hospice. You will conduct an initial psychosocial assessment of the patient/caregiver contributing to the Agency''s plan of care. The

To provide medical services to Hospice patients and/or family members reflective of the philosophy, mission, and standards established by the Agency.What we’re looking for in you:

Current MA license in good standingBachelor''s degree in counseling or closely related fieldMaster of social work from an accredited program (MSW)3 years + experience of social work experienceHospice work a plusPrevious community health and medical health experience preferredIn-depth knowledge of social service skills and resourcesGood communication skills, both verbal and writtenAbility to harmoniously interact with patients/clients, families, and colleaguesHandle pressure in various work environments and work in the fieldAbility to work independently with minimum supervisionExcellent organizational, flexibility, and time management skillsKeeping up to date with education, policies, and proceduresAbility to cover the full South Shore field and in the future focus on one central areaFlexibility for on-call holiday and weekend rotationWhat''s in it for you?
