Our community services include health visiting, school nursing and support for families of children with long-term conditions.Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. This is an exciting opportunity to lead a high-profile transformation programme that will shape the future of pharmacy homecare services across South East London. Working jointly with Guy’s and St Thomas’ NHS Foundation Trust and the South East London Integrated Care Board, the Senior Project Manager will deliver a system-wide programme to modernise, harmonise, and sustain homecare medicines services.The postholder will play a pivotal role in improving patient access, ensuring financial sustainability, and developing a collaborative SEL homecare model. This role will work across organisational boundaries, engaging with Chief Pharmacists, clinical teams, finance and commissioning colleagues, NHS England, community pharmacy, homecare providers, and patient groups to deliver lasting system change.Main duties of the job

The Senior Project Manager will lead the South East London Pharmacy Homecare Transformation Programme, providing overall programme management and delivery across multiple Trusts and system partners.Establishing programme governance, reporting, and stakeholder engagement structures.Leading prescribing harmonisation across Trusts and supporting pilots such as EPS/CLEO prescribing into community pharmacy.Developing and delivering a transformation roadmap for a collaborative or centralised SEL homecare function.Ensuring robust financial planning, tracking VAT savings, and identifying cost-effective supply models.Coordinating with pharmacy clinical teams, finance, commissioning, NHS England colleagues, and patient representatives to deliver sustainable change.Overseeing development of performance dashboards and KPIs to ensure delivery against key milestones and system benefits.Ensuring programme delivery complies with NHS legislation, governance frameworks, and Trust policies.The role requires strong partnership working, programme management expertise, and the ability to engage senior stakeholders to achieve system-wide improvements.Working for our organisation

The Senior Project Manager will be responsible for leading the South East London (SEL) Pharmacy Homecare Transformation Programme, working jointly with Guy’s and St Thomas’ NHS Foundation Trust and the SEL Integrated Care Board. The programme will deliver a sustainable, consistent and collaborative approach to pharmacy homecare services across SEL, improving patient access and ensuring financial and operational resilience.Key responsibilities include:Programme leadership

– establish and manage governance, reporting and stakeholder engagement structures; provide assurance on delivery to GSTT and SEL ICB leadership.Prescribing harmonisation and service redesign

– coordinate standardisation of prescribing processes across SEL Trusts; support pilots such as EPS/CLEO prescribing into community pharmacy; and progress the development of a centralised SEL homecare function.Financial management

– ensure robust business cases, monitor budgets, track VAT savings and cost efficiencies, and oversee procurement and commissioning processes that deliver value for money.Stakeholder engagement

– work collaboratively with Chief Pharmacists, pharmacy clinical teams, finance and commissioning colleagues, NHS England, community pharmacy, external homecare providers, and patient groups to co-design and deliver sustainable models of care.Performance and evaluation

– oversee development of performance dashboards and KPIs; ensure risks and interdependencies are managed; monitor delivery against agreed milestones; and embed lessons learned into programme improvement.Strategic leadership

Qualifications / Education

Relevant MPharm or equivalent professional permitting qualification as a Pharmacist or Pharmacy Technician with the GPhCRegistration with the General Pharmaceutical Council (GPhC)Postgraduate qualifications in Clinical Pharmacy Practice (PGCert, MSc or equivalent)Member of relevant professional groups (RPS, UKCPA)Leadership or management qualification (or underpinning knowledge)Knowledge and Experience

Experience of working with clinical specialities that actively rely on homecare services to provide medicines to patientsKnowledge of the homecare commissioning landscape and how homecare services support NHS careKnowledge of the wider NHS landscape and how introduction of ICBs / ICS structures influence service design and deliveryEvidence of leading successful project and / or operational performance and improvement; including business case preparation, service initiation / commissioning, contract negotiation, and the development and monitoring of output and outcome measures.Experience of managing organisational change and re-designing services.Extensive project management experience in large complex organisations, including design, development, implementation and management of change in a multiple stakeholder environment.Skills and Abilities
