This is an exciting opportunity for a pharmacist committed to delivering high-quality, patient-focused care across the system.Key ResponsibilitiesScreening medication charts, discharge prescriptions, and one-stop orders to ensure safety and effectivenessMedicines reconciliation within 24 hours of admission and at dischargeInterpretation of blood results in relation to medicines and adjusting therapy accordinglyRespond to ward-based medicines information (MI) queriesAttend and contribute to MDTs and consultant ward roundsCheck controlled drugs (CDs) and perform regular ward auditsOrder medicines and liaise closely with Medicines Optimisation TechniciansSupport the safe transfer of patients across care settings, ensuring complete and accurate discharge summariesContribute to antimicrobial stewardship and public health campaigns (e.g. flu vaccinations)Skills and Systems Experience (Desirable)Working knowledge of SystmOne (S1), SeeEHR, ICM, and pharmacy dispensing systemsProficient in Microsoft Office applicationsAbility to handle complex medicines queries using clinical databases, protocols, and national guidelinesCore Competencies and ExpectationsApply clinical reasoning and judgement ..... full job details .....
