The Wandsworth Hospital Team works in close partnership with the Wolfson Unit to deliver a high-quality, person-centred discharge service that promotes independence and well-being for our residents and their carers. We are looking for someone with demonstrable expertise in supporting individuals with complex physical and cognitive needs, and who can confidently navigate the challenges of longer-term care planning. As a Senior Social Worker, you will be expected to lead on complex casework, provide supervision and guidance to team members, and contribute meaningfully to service development and performance improvement.
