An exciting opportunity has arisen for Band 7 Clinical Leads to join our Neighbourhood Mental Health Team working across South Birmingham, in our Primary Care Networks (PCNs).We are looking for enthusiastic and dynamic individuals who are able to work within a multi-disciplinary team, alongside other professionals, striving for the best outcomes for individuals within the local community that the team serves. Our community mental health services are well-established teams; as part of the BSOL community transformation programme, we are increasing our neighbourhood mental health teams with these roles, focusing on a presence alongside GP colleagues within PCNs. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care. The development of our Neighbourhood Mental Health Team is an essential service evolving in line with the NHS Long Term Plan.Welcome to Birmingham and Solihull Mental Health NHS Foundation Trust. Provide mental health triage, assessment and outcomes-based intervention for service users within the Neighbourhood Mental Health Team.Identify need and determine the best course of advice, support or intervention.Promote psychosocial wellbeing by identifying and supporting pathways for service users and carers.Ensure robust communication across FTB/BSMHFT and primary care in accordance with each individual service user’s needs by providing support and advice to all colleagues with the PCN as required, including participation within the local neighbourhood MDTs and those working in the voluntary sector.About the Trust and Service
