Job Purpose:To provide high-quality, person-centred support to individuals who are homeless or at risk of homelessness, helping them to sustain tenancies, build independence, and move on to longer-term housing solutions. You will work collaboratively with referral agencies and support networks to deliver tailored support and housing-related services.Key Responsibilities:Engage with actual and potential referral agencies (e.g. local authority gateway services, statutory/voluntary agencies) to generate and manage referrals.Develop and deliver personalised support and safety plans, referring to specialist agencies where appropriate.Record, monitor, and regularly review support plans, ensuring service user contact and outcomes are tracked in line with contractual requirements.Ensure properties are properly equipped, maintained, furnished, and cleaned to a high standard.Issue and manage tenancy or licence agreements, ensuring service users understand and comply with expectations.Take appropriate action in cases of non-compliance or disengagement, including partnership working or enforcement as needed.Empower service users to set and achieve realistic goals through regular engagement and motivational support.Facilitate service user involvement in decisions that affect them, promoting independence and responsibility.Identify move-on options early and support clients to develop sustainable housing pathways.Support service users in developing life skills and completing domestic ..... full job details .....
