Community Mental Health Practitioner (CPN)

OverviewGuildford Community Mental Health Recovery Service (CMHRS) is looking for an experienced and enthusiastic community mental health nurse (CPN) to join our team. The successful applicant will deliver comprehensive assessments and a variety of approaches, interventions, and treatments to individuals experiencing severe and enduring mental health issues, supporting them in their personal recovery journey. Travel across the trust is essential due to the community-based nature of this role.
Responsibilities
Take lead and professional responsibility for a defined caseload of service users.
Communicate effectively and work collaboratively within a multidisciplinary team.
Deliver a high-quality service, working independently and as part of a recovery-focused team to meet the needs of service users and their carers.
Conduct initial assessments for clients referred to the service.
Support individuals experiencing mental health crises in the community.
Engage in various training opportunities to enhance skills in delivering low-intensity psychological interventions.

RequirementsQualifications
Qualified RMN/RN (Mental Health)
NMC Registration
Experience
Demonstrable post-qualification experience working with individuals with mental health needs in the community
Aware of personal and professional boundaries
Evidence of continuous professional development
Clinical supervision training
Desirable
Successful completion of Mentorship course
