Responsibilities
In the role of Care Navigator you will work as part of a cross-city team, providing a professional and effective response to our patients who access our services. This will be online or over the telephone and you will be responsible for signposting patients correctly to ensure they receive the right care, at the right time, with the most appropriate healthcare professional for their individual care needs.
