Formulation of person-centred care plans; review and update of care plans to reflect client need, ensuring plans are completed and maintained in consultation with clients, relatives and other health care professionals, in accordance with NMC guidelines and company policiesAdministration of prescribed medications and documentation in accordance with the company’s procedure and NMC guidelinesMonitoring of resident well-being; report ill health and arrange GP/professional visits as necessarySupervision of the care team when acting as nurse in charge; provide clear handover to staff at end of shiftLiaise with healthcare professionalsApplication Process
