We work closely with individuals, their families, and community partners to build personalised support plans that focus on what matters most to them.As an Experienced Social Care Practitioner, you''ll carry out assessments, develop support plans, and review outcomes in line with the Care Act 2014. You''ll work with people to understand their strengths, identify risks, and connect them with local resources. You''ll also play a key role in safeguarding, supporting carers, and promoting wellbeing.You''ll need experience in a health or social care setting, strong communication skills, and a good understanding of relevant legislation.
