Housing Support Worker

Job Summary We are seeking a compassionate and dedicated Support Worker to join our team. The successful candidate will play a vital role in providing assistance and support to individuals, particularly children, in various settings. This position requires a commitment to enhancing the quality of life for those in care while promoting independence and well-being. Duties Assist individuals with daily living activities, ensuring their comfort and safety. Develop and implement personalised care plans tailored to the specific needs of each individual. Communicate effectively with clients, families, and other professionals to ensure continuity of care. Provide support in childcare settings, fostering a nurturing environment for children. Drive clients to appointments or activities as needed, ensuring safe transportation. Maintain accurate records of care provided and any changes in the individual''s condition. Collaborate with multidisciplinary teams to enhance service delivery and client outcomes. Skills Strong communication skills, both verbal and written, to engage effectively with clients and their families. A valid driving licence is preferred for transporting clients as required. Proficiency in IT skills for maintaining records and utilising care management software. Experience in developing and implementing care plans that meet individual needs.
