Sutton Primary Liaison Recovery Service (PLRS) includes a Single Point of Access (SPA) and a Primary Care Recovery Team. Sutton SPA screens, triages and provides assessment for working age adults aged 18 - 74 presenting with serious and/or enduring mental illness and risk deemed too complex to be managed under Primary Care Services. Following assessment clinicians make appropriate onward referrals to internal and external teams with recommendations and signposting. We work closely with other teams to facilitate appropriate transfer of service users to internal teams within the trust including Adult Community Teams, Acute Care Pathway Services and Teams, Specialist Services and Taking Therapy Services. We also work in close partnership with GP commissioners and other key partners, including voluntary and third sector organisations.Responsibilities
Carry out comprehensive specialist assessments ensuring discharge from the service is clinically sound and timely, escalating for further clinical advice when appropriate.Make clear autonomous decisions based on analysis of complex presenting problems and options related to admission and discharge of patients.Provide specialist consultation and support for Band 6 practitioners, assisting in decision making in difficult and complex cases.Support triage of complex calls and escalation whilst carrying out the duty function.Coordinate resources across the service to meet pressing needs and step down into other functions when clinically and operationally required.Work with colleagues to develop and promote the service philosophy, framework of care delivery and strategic objectives; take responsibility for policy and strategy delivery in specific areas.Develop clinical expertise and high standards of nursing practice as defined by the Nursing and Midwifery code of professional conduct (2015).Deliver care that is evidence-based, reflecting current best practice and research.Participate in structured clinical supervision per Trust policies and procedures and supervise nurses and others in non-mental health settings working with patients with mental health problems.Demonstrate clinical excellence and high standards of nursing practice to other staff.Demonstrate understanding of legal and ethical issues in caring for the mentally ill, including Mental Health Act (1983) and Mental Capacity Act and their use in non-mental health settings.Qualifications

EssentialClinical management and leadership experience; experience working with primary and secondary care, local authorities and voluntary organisations; ability to communicate sensitive information; knowledge to ensure compliance with statutory guidance and best practice; experience of working with adults with serious ongoing mental health problems; experience in recruitment and selection of staff; experience implementing Recovery principles.DesirableNon-medical prescriber or willingness to undertake training to become one; experience implementing changes in working practices; experience using and supervising psychosocial interventions.Knowledge and Skills
Contribute to, monitor and implement changes to services; develop policies and guidelines compliant with national and clinical standards.Comprehensive knowledge of national guidance and best practices related to acute and urgent mental health care.Ability to communicate highly complex information to groups; strong negotiation and influencing skills; excellent verbal and written communication; ability to respond rapidly to changing situations.Details
