qualified
and
experienced
social worker to join our friendly and supportive team. This is a busy team based in the community and consists of Social Workers and Social Care Practitioners who work closely with partner agencies. The successful applicant will be able to demonstrate the skills, knowledge, and experience to undertake this role to provide a safe, accountable, cost effective and efficient service to help people using our service achieve their outcomes.Please see the job and person profile for more information.There is an expectation that you will act as duty worker on a rota basis which will involve triaging new referrals to the team and supporting clients and carers who require a more urgent response. Carry out Care Act 2014 assessments and reviews for individuals with complex needs and arrange provision of services to meet eligible needs.Work in a person-centred and strengths-based way with people and their families to maintain and maximise independence.Demonstrate safeguarding understanding and experience; undertake capacity assessments in accordance with the Mental Capacity Act 2005.Act as duty worker on a rota basis, triaging new referrals and supporting clients and carers requiring an urgent response.Collaborate with health partners and other agencies; manage change effectively; build positive relationships.Qualifications and Experience
Qualified social worker (with current registration).Experience in Mental Health teams and community-based practice.Experience in safeguarding and Mental Capacity Act applications.Benefits
