Facilitate key work sessions, reviews, and meetings in line with support plans. Provide advocacy, advice, and support to ensure service users meet occupancy conditions. Work in partnership with external agencies to maximise opportunities for supported individuals. Manage a personal caseload while contributing to the wider team. Support referrals, admissions, and discharges in accordance with procedures. Maintain a safe environment through regular risk assessments, health and safety checks, and cleaning duties. Help individuals with tenancy management, budgeting, and accessing services. Uphold data protection and confidentiality requirements while maintaining accurate records.
