This is a fantastic opportunity to work within a fast-paced, multidisciplinary environment, supporting timely hospital discharges and community-based care planning. About the Role: This role will be primarily hospital-based, with a focus on supporting individuals to return home safely with the right care and support in place. Based 5 days per week (Monday-Friday) Primarily located at Morriston Hospital Potential to cover Singleton Hospital, Neath Port Talbot, and Gorseinon Combination of hospital and community-based work What We''re Looking For: We are seeking a qualified social worker who is confident working in a fast-paced setting, with experience in hospital discharge, safeguarding, or community-based assessments.
