You will work as part of the Children’s Community Nursing Team and Children’s Nurse Specialist Team, supporting and managing children within the community and in hospital with diagnosed respiratory conditions.Responsibilities include assessing, planning, implementing, and evaluating care to support children/young people in remaining in the community and preventing hospital admissions where possible. When hospital admission is necessary, you will support the inpatient ward team to meet the child''s needs and facilitate timely discharge.You will collaborate across local and national networks for asthma, cystic fibrosis, long-term oxygen therapy, tracheostomy, and home ventilation management.Participate in service development and work collaboratively across health, social care, and multi-agency teams for patients with complex respiratory conditions.Manage your own caseload autonomously, providing specialist advice and support to children, young people, and their families to improve self-management in community, hospital, and nurse-led clinics.Support unwell patients through appropriate triage, visits, and care planning, empowering them to manage their condition, recognize deterioration or acute episodes, and access urgent support.Ensure nursing practice complies with the NMC/HCPC Code of Professional Conduct and complete role-specific competencies.Join us at a pivotal time as we build a new state-of-the-art hospital opening in 2031/32 and undertake significant digital transformation, including the implementation of a new electronic patient record system from 2026 to enhance coordinated care.We provide a comprehensive range of services to approximately 331,000 people across west and north Norfolk, North Cambs, and South Lincs.
