HCC Services for Young People provides informal education / youth work, careers education information advice guidance, work related learning, work experience and the review and maintenance of education health and care plans for young people post-16 / post school. Our vision for young people is clear. We want young people to succeed, and by working for us you''ll be key to enable this to happen. We work with vulnerable young people to address emerging needs, improve life chances, and reduce escalation to more expensive and intensive services. This includes supporting those facing challenges in formal education, their community or home life - to ensure a successful transition to adulthood. Young people are typically 11-17 and up to 25 for Care Leavers and young people with Special Educational Needs and Disabilities. You''ll be delivering important targeted work including evening group work projects, supporting those missing from education to reengage successfully back into school or college and work with young people with SEND. You would be giving young people support at an early stage, preventing their issues from seriously affecting their daily life, health, development, or safety. We are looking for candidates who can quickly establish positive relationships with diverse groups of young people and respond flexibly to their needs and the priorities of the service.
