About the Role Empower a caseload of service users to manage their health and wellbeing. As part of a team, you will support a staffing rota offering support to service users between the hours of 8am-10pm Mon-Sun each week. In collaboration with your team, you will actively monitor wellbeing, mental health, housing and living environment to ensure service users journey into independence. Moreover, you will take a solution focused and co-produced approach to supporting service users with their move-on into independent living. Sounds great, what will I be doing? This role involves empowering service users to build independence and engage confidently with professionals in the community, such as mental health teams, social workers, housing providers, and employment or training services. You will support continuous assessments of service users'' needs, advocate for appropriate support levels, and help guide them through transitions in their journey toward independence.
