We are looking for an experienced and enthusiastic Team Leader to join our dedicated team of staff at our Salford Physical Disabilities Service. As a Team Leader, you will work closely with the Registered Manager and Deputy Manager ensuring the effective delivery and daily operational management of our service. You will be responsible for leading a team of staff, providing supervision, coaching, performance management, and support to the staff team. You will ensure the provision of person-centered support that enables customers to maintain their independence, enhance their wellbeing, and enjoy opportunities for personal development. Your role includes overseeing the deployment of staff to meet individual needs and preferences within available resources, identifying training needs (both mandatory and service-specific), and maintaining accurate records. You will respond to referrals, undertake initial assessments of need, and ensure all service users have co-produced support plans and designated Key Workers. Establishing clear goals and pathways towards greater independence for service users, you will organize regular person-centered reviews, putting each person''s views and journey at the heart of planning and decision-making. Working closely with local multi-disciplinary teams and community-based agencies, you will provide holistic, wrap-around support.
