0-19 Service-Public Health Practitioners Health Visitor/School Nurses role at Croydon Health Services NHS Trust. The 0-19 enhanced pathway is an integral and distinct caseload within the Croydon 0 to 19 Public Health Nursing (PHN) service. A programme of care will be delivered to families and young people with additional health needs and vulnerabilities determined by inclusion criteria. The aim is to improve health and development outcomes of babies, children and young people and their families. The tools used by the Enhanced Pathway Nurses (for example the Family Partnership Model and Outcome Star) are underpinned by research to get the best outcomes for children, young people and families. We are seeking qualified Health Visitors and School Nurses who are eager to enhance their leadership capabilities and contribute to the future direction of Public Health Nursing in Croydon. The role supports the Healthy Child Programme and safeguarding policies and involves collaboration within a multidisciplinary team to deliver the Healthy Child Programme and provide added support to families with children aged 0-5.Responsibilities
Lead and deliver the Healthy Child Programme (HCP) for a designated and defined caseload of families; act as the named health professional for planning and delivery of the HCP, reporting to and being line managed by a Team Leader or Clinical Service Lead (CSL).Plan and manage own workload to meet client needs, clinical activity targets and KPIs.Work in partnership with team members, infant feeding colleagues, service users, Croydon Health Service staff, and other stakeholders to promote the health and wellbeing of Children, Young People (0-19 years) and their families.Collaborate with the CSL, Practice Educator and multidisciplinary teams to develop innovative clinical practice aligned with the Healthy Child Programme.Assess client needs using tools such as Family Strengths and Needs, Early Help Assessments and Risk Assessments to plan, implement and evaluate evidenced-based care, and coordinate with other agencies when care involves chronic illness or disability.Liaise effectively with colleagues within Croydon Health Services and partner organisations to ensure timely, well-coordinated care for children and families.About Croydon Health Services NHS Trust
