This is a key clinical role supporting patients admitted with drug or alcohol related needs, ensuring they receive the right support in hospital and a smooth transition back into community treatment.This is an exciting opportunity to work at the intersection of acute healthcare, substance misuse treatment, and public health, helping to reduce harm and improve long-term outcomes for vulnerable patients.In this role you will:Complete comprehensive clinical assessments and risk evaluations for patients with substance misuse needsSupport safe alcohol and opioid withdrawal management alongside hospital medical teamsLead on Hepatitis C screening, testing and referrals into treatment pathwaysDeliver harm reduction interventions including brief advice, BBV awareness and naloxone provisionDevelop effective discharge plans to ensure continuity of care between hospital and community servicesRefer patients into detoxification, treatment programmes and structured recovery supportConduct Fibro Scans and ECGs as part of holistic clinical carePartnership working is central to this role. ..... full job details .....
