Overview

We achieve this in two ways. First we improve lives on the frontline through delivery of public services. Secondly, we use our knowledge to change ''the system'', to fix the complex web that can trap and disempower those it was set up to help. Our Young People and Families (YPF) hub provides early intervention, targeted and specialist support services to those in crisis, leaving care, missing from home or with substance misuse or mental health problems. Our services support people no matter what their situation - and have demonstrated a high success rate.Responsibilities

Provide one-to-one tailored support to young people, working collaboratively as part of a multi-agency team to ensure safety and maximise outcomes. Tailor support to each young person’s individual needs and circumstances; work as part of the Home Office-funded County Lines Support and Rescue Service.Access to a car and a full current driving licence; provide end-to-end support and interventions to a pre-defined caseload of young people and their families involved in County Lines activity, with referrals via effective assessment, care planning and risk management.Proactively assess and manage caseload risks, escalating where necessary through safeguarding procedures, and adapt case management frequency and approach to reflect individual needs.Ensure diverse needs and aspirations of all service users are considered in service planning and delivery to enable them to fulfil their potential.Liaise effectively with local partner organisations and other relevant agencies to ensure referrals, care, risk management, and smooth transitions for young people.Maintain excellent verbal and written communication with partner agencies and within the team; demonstrate honesty, reliability, creativity and a flexible approach to work.Deliver training to young people and professionals, in both group and one-to-one settings; maintain high attention to detail and apply risk assessments and engagement strategies for young people with complex needs. Use ICT systems for recording and data collation as required.Work with young people and families identified as vulnerable or with complex needs, preferably in a community setting; engage in inter-agency working, preferably with Children’s and Family Services/Youth Justice Services/Youth Services. Maintain knowledge of risk and vulnerability factors affecting young people.Desirable: possess a relevant qualification in youth, community, health and social care, or management.Requirements

Experience of working with and delivering training to young people and professionals, in group and individual settings.Good ICT skills and experience of data recording and data collation systems.Experience of working with young people and their families identified as vulnerable or with complex needs, preferably in a community setting.Experience of inter-agency working, preferably with Children’s and Family Services/Youth Justice Services/Youth Services; knowledge of risk and vulnerability in young people.About Catch22
