Health and wellbeing practitioner - HMP Coldingley - Bisley, Woking. Permanent role with a salary of £25,714.29. The Forward Trust is recruiting a Health and Wellbeing Practitioner to join the team at HMP Coldingley, a closed Category C male prison in Surrey. You will work within an integrated healthcare framework alongside a multi-disciplinary team to deliver clinical and psychosocial interventions to adult male prisoners, including individuals with low-level mental health needs and/or substance misuse issues.Responsibilities
Managing a caseload and delivering tailored recovery-focused interventionsConducting assessments and co-producing SMART care plans with service usersFacilitating structured 1:1 sessions and group programmesProviding harm reduction advice and practical recovery supportSupporting release planning and ensuring through-care into community servicesPerforming second signatory duties as neededMaintaining accurate, up-to-date case records in line with service KPIsThis is a 7-day-a-week service, so weekend working will be required on a rota basis.What We’re Looking For
