OverviewCNWL are pleased to announce a vacancy within Hillingdon Single Point of Access (SPA) and Palliative Responsive Nursing Service YLL 24/7. We are looking for an innovative, enthusiastic and dynamic nurse to join our team whose aim is to improve coordination and continuity of care for patients and their families, and to provide responsive in- and-out-of-hours nursing care in the community for those at the end of life. Our goal is to introduce a single contact telephone number to enable direct access to services and support for health and social care professionals, patients and their families/carers.

You should have a creative, imaginative and flexible approach to service delivery and quality improvement, and excellent interpersonal skills with postgraduate experience in the community, hospital or hospice setting with palliative care experience. Flexible working hours are available. This is a 24-hour service requiring day and night shifts. Car user is essential.

Responsibilities

Improve the coordination and continuity of care and support for patients and their families/carers.

Triaging telephone calls into the service from patients, carers, relatives and clinicians.

Triaging referrals into the service.

Enable responsive in- and out-of-hours nursing care for people in the community who are at the end of life, providing high-quality end-of-life care.

Work closely within a multi-disciplinary team to ensure excellent communications.

Hospital admission avoidance.

Uphold Advance Care Planning (ACP) wishes and support patients to achieve their desired place of care/death.

Working for our organisationYLL 24/7 are a small team of CNSs, HCAs and admin staff who strive to provide the best possible care to palliative/end-of-life patients in their own homes in the community. We pride ourselves on great teamwork and rising to the challenges of day-to-day work in a diverse community. No two shifts are the same with YLL 24/7, which makes it challenging but rewarding.

Note: This role may involve contact with patients or service users. The NHS strongly encourages vaccination to protect patients and colleagues.

Person specificationQualifications

RGN [NMC Registered]

Relevant post-basic qualifications

Recognised teaching qualification

Achieved or working towards a first degree

Current driving licence

Desirable qualifications

Non-medical prescriber and physical assessment qualification

Skills / Knowledge

Contemporary and relevant clinical knowledge and expertise

Ability to work independently and as part of a team

Basic computer literacy

Ability to negotiate with a variety of clinical professionals, patients and their families

Ability to prioritise workload and manage time effectively

Problem solving and action orientation

Experience of working with integrated care pathways

Ability to supervise and develop people

Experience

Substantial post-registration experience

Extensive experience in an appropriate speciality

Experience of case/ward management

Experience of teaching clinical staff and patients and families

Experience of caring for patients in their own home

Audit and Research

Previous experience of audit, standard setting and quality issues

Ability to incorporate research into practice

Professional Requirements
