An exciting opportunity to join an innovative and dynamic team providing intensive rehabilitation in the community. We are looking for an experienced and motivated Occupational Therapist who is passionate about service development and delivering high-quality, patient-centred care. You will work closely with a friendly team of physiotherapists, occupational therapists, nurses and rehabilitation assistants who are committed to maintaining independence, promoting self-care and helping patients achieve their goals.The Virtual Frailty Ward / Intensive Rehabilitation Service provides daily therapy in the patient\''s own home. We prioritise our caseload to respond to changing daily needs and communicate closely with other service providers to avoid admission where it is safe to do so, providing onward referrals to other community services including health and social care. As part of the Frailty Virtual Ward / Intensive Rehabilitation Service (IRS) we see a wide variety of patients including neurological, orthopaedics, falls, long-term conditions, and musculoskeletal issues, enabling staff to gain broad experience across specialties.You will be part of a friendly, approachable team who work well together and will be supported by a strong senior management team. This role is based within a sponsor organisation with certain eligibility requirements under the UKVI points-based system.Responsibilities

Provide daily therapy in patients\'' homes as part of the Virtual Frailty Ward / Intensive Rehabilitation Service.Collaborate with a multidisciplinary team to support independence, self-care and goal achievement for service users.Prioritise caseload to respond to changing needs and make safe onward referrals to community services.Work flexibly to respond to service needs and demonstrate initiative.Engage in service development and continuously seek to improve patient-centred care.Qualifications and Experience

Qualified Occupational Therapist with relevant experience in rehabilitation and community settings.Committed to high standards of patient-centred care and service development.Ability to work effectively within a friendly team and within a supportive management structure.Benefits

The Virtual Frailty Ward / IRS operates seven days a week, 365 days a year. Therapy is mainly 9am to 5pm, Monday to Friday, with flexibility to accommodate needs. Weekends are covered on a rota basis (usually one weekend per month, 9am to 12:45pm, Saturday and Sunday). Service areas include Brentwood, Billericay, Wickford, Basildon and Thurrock.
